I have been low carb, paleo but with dairy, for 8 yrs. 5 yrs ago fasting glucose was 4mmol/L and insulin was 2 mIU/L. HbA1c was 5.3 & 5.
Recent HbA1c is 5.7% so I bought a keto-mojo meter and can see my glucose readings are higher than I’d like and appropriate for that level of 5.7%. Where am I going wrong? I’ve heard of ‘physiological insulin resistance’ on low carb diets but I have too much glucose sloshing around in my blood and don’t know how to lower it.
My theories so far…..I’ve put on weight just due to overeating ( subcutaneous) so have to lose it to correct glucose.
Have I eaten too much protein in an effort to try and build muscle ( along with resistance exercise)?
I’ve had to double my progesterone part of my HRT and now my body thinks it’s pregnant and is insulin resistant ?
I’ve started fasting each week, between 1-3 days and after about 24 hrs my glucose levels come down to a better level. I managed ketones of 1 at 1 day, 2-3 on day 2 and 3-4 on 3rd day.
I know right? mine def trended up on keto, proper very low carb, ( still had dairy from memory, I have a diary of alot of that year), when I was finger pricking regularly.
more recently I did a cgm for a couple of months.
theories-
menopause / peri ( now done)
coffee
dairy
physiological insulin resistance ( or rather glucose intolerance as dr ben bikman calls it)
I stopped measuring, and was carnivore for best part of a year recently.
I started with a bit fast ( 10 days, but I was in the habit of 3 days or so, I used acv, pink salt and fizzy water, and black coffee).
because I am thru menopause I don't get a cortisol spike it seems, and anxiety with fasting.
I'm a sugar addict, and I'm struggling lately- but when I was carnivore ( meat fish fowl eggs, even without nuts seeds berries veg, & no dairy in my case)...my
trigger thumb ( arthritis from
hypermobility) reversed and
healed.
I had fewer ( or no) hypermobility pain flares.
I am assuming glycation is avoided and gluconeogenesis takes place as and when needed.
also if there is fat to lose ( as in my case, although I'm 30 rather than 70 lbs over my aim...) I think that as the fat is burned, sugar is released?
so in the fat burning process, the sugar is sequestered away with the fat stores, then free fatty acids are burned if you are metabolically flexible and need energy ( ie can burn either fat OR glucose as necessary), but sugar is being released into the bloodstream too?
Was reading this excellent substack paper and listening to CNBC. One of the panelist mention an FDA approved AI assisted diagnosis of diabetic retinopathy. Missed the company name but search found this:

Why isn’t being low carb enough?
Thanks!

this could raise blood glucose..
